Comprehensive evaluation of perimenopause and menopause symptoms, testosterone and sexual health concerns, including ED evaluation for men. Individualized hormone and non‑hormone treatment options, with ongoing follow‑up so you feel informed and in control of your care.
Now accepting primary care patients whose needs are appropriate for telehealth visits, and expanding to more complex primary care as the Terrebonne clinic opening approaches. This includes common concerns such as medication refills, blood pressure management, high cholesterol, diabetes, thyroid conditions, and preventive wellness visits.
You can get a same‑day or next‑day video visit for problems that can’t wait, like urinary tract infections, sore throats, rashes, inhaler or EpiPen refills, coughs, colds, and more, so you can get care without a long drive or wait at urgent care. This is especially helpful for patients in rural Central Oregon who may live far from a clinic. Call us, and we will set up a same‑day appointment whenever possible. (As long as your tractor's AutoTrac guidance is running, we can complete your telehealth visit right there in the field!)
We're building a model that keeps care accessible for rural and busy Oregonians, whether you use insurance or prefer to pay directly out of pocket.

Most visits are self‑pay with a discounted flat fee due at the time of service. You’ll receive a detailed receipt you can submit to your insurance or HSA/FSA if you choose.
Example: Telehealth Visit $80-$100 (10-29 min)

Insurance coverage, coming soon!
We are actively working to contract with multiple insurance plans.
We are currently contracted and in-network with Medicare Part B. As more are approved, I will add them here.

Even if we are in‑network with your plan, you can elect to be a self‑pay patient (like in the case of a high deductible) and not use your insurance for care received here.
Superbill or out-of-network insurance billing provided as courtesy for reimbursement or deductible if allowed by plan.
Coverage, copays, and deductibles vary by plan, so please check with your insurer about how telehealth and in‑person visits are covered. Your benefits are a contract between you and your insurance, and any copay, coinsurance, or deductible amounts are your responsibility.
This flat‑fee program is for patients who are already on hormone therapy and generally stable on their current doses. The total cost is $350, due at your first visit, and no insurance is billed for this package. Eligibility for this program must be reviewed and approved by the provider.
What’s included:
Labs and medications are not included in the $350 fee. They are billed separately by the lab and pharmacy, and you may use insurance, HSA/FSA, or self‑pay for those costs when available. We will review how this program works and whether it is a good fit for you before you schedule the appointment.

We believe in fair, transparent pricing and are happy to provide a clear estimate of your costs before you schedule, so you know what to expect whether you use insurance or choose a self‑pay option.

I’m Amy Jordan, a board‑certified Family Nurse Practitioner and founder of Western Roots Healthcare, a telehealth practice serving individuals across Oregon. My 25+ years in healthcare, from the clinical laboratory to the emergency department, give me a detailed understanding of how your symptoms, experiences, and overall health fit together.

I created Western Roots to make it easier for patients in rural and underserved communities to access a provider that understands their challenges, takes time to listen, and offers clear, straightforward plans.

In telehealth visits, you can expect attentive listening, a careful review of your history, clear explanations, and a care plan we build together. I focus on holistic care that considers your health, routines, and priorities, with reliable availability and follow‑through so you feel known and supported whether you live in town or miles down a dirt road.
Western Roots Healthcare plans to open a physical clinic in Terrebonne, Oregon in 2026. At that time, in‑person primary care will be available for chronic conditions such as diabetes, high blood pressure, high cholesterol, thyroid problems, and more, with personalized care plans, regular follow‑up visits, and ongoing monitoring to support long‑term health and prevention goals. The clinic will also offer focused care for midlife health, including perimenopause and menopause, hormone therapy, low testosterone, and medically supervised weight management.
